Couple of repeat questions here

I've seen these two answered before but can't recall the specific answer.

First, does the disease normally slow down a bit with advancing age? I just turned 63 so my question would be does the tendency for DD to recur or extend slow down, say by age 70 or so. My recollection is that it does but not sure at what age.

The other one is that if you have open surgery is the recurrent contracture normally less than the original. And it that also true for NA or collagenase?

Another one for Wolfgang perhaps? But in the Annet L. van Rijssen, et al, paper "Five-Year Results of a Randomized Clinical Trial on Treatment in Dupuytren’s Disease: Percutaneous Needle Fasciotomy versus Limited Fasciectomy" they found declining recurrence after surgery or PNF in older age groups.

My sense is perhaps it slows down but not probable. My thinking is that age is one of those factors that causes people to get DD and that for those that get it later in life the progression is slow.

Alternatively it is possible that the disease starts slowly and gets faster but this would appear to be the same as people who get it first in their 60's might fall off the perch before it becomes a problem.

On recurrence I don't think there is any doubt that with NA the recurrence is more rapid than with surgery but NA is a 20 minute procedure with no recovery time required and can be repeated as often as needed while surgery is a lot more serious and there are issues with scar tissue that limit the repeat option. I think of NA as being plaque removal and surgery as being root canal or a crown.

Hi Tusk, you might have a look at the figure at the bottom of http://www.dupuytren-online.info/NA_side_effects.html, which is from the paper that Spanishbuddha is referring to. It shows how the recurrence rate of NA drops with age. A good news when getting older!

Regarding the amount of contracture after recurrence I can't recall a paper studying this. The amount of recurrent contracture most likely also depends on time. The longer you observe, the more contracture.
