Dupuytren - When to have surgery!

I was first diagnosed with Dupuytren some 10 to 15 years ago, when the only symptom that I can recall was pitting of the right palm. In the intervening years, I have NOT suffered from any pain or curling of the fingers. However, starting 2 months ago I have started to experience pain in the right palm at the base of the index finger and pain in the finger itself. This pain is more intense if I press on the source and when washing my hands and face and the index finger is then pushed backwards, or whenever the finger is deflected backwards. In addition, over the last couple of weeks, when I uncurl the finger it does so in a series of "flicks" at each joint, which I think is referred to as "trigger finger".

So my question is: When should I consider surgical intervention, if at all?

I would be most grateful; for any advice that anyone can offer. I am based in Bristol, UK.

Doesn't really sound like DD, more a tendon inflammation that might lead on to full trigger finger, unless it's a DD nodule. In either case don't rush into a surgical option, a conservative approach might help, maybe a steroid shot, but do see a specialist for advice, there's a list of Dr's on the BDS website.

Thank you spanishbubba, but I have had a GPs diagnosis of Dupuytren, initially 10-15 years ago and latterly about 1 month ago. However, GP also told me that I could not have treatment on NHS. Hence considering private treatment but only if thought adviseable at this stage.

The difficulty is unless you have a contracture the only approved treatment on the NHS is RT, and that is a problem for many, but not all, CCG’s or Trusts. There is a list on the BDS site, but you would need the referral, and the conditions have to be right.

In my previous post I was referring to the jerky finger movement you described. It is unusual for DD to cause that symptom, not impossible perhaps, but I was suggesting you get that symptom diagnosed and treated. It may still be caused by DD, in which case please return and let us know.

I got my hand caught in I printing press. TWICE back in the 80’s. Brlllent. Ha. I had the best doctors in the entire world. James Hunter ....Jefferson Hand center in Philadelphia. He says to me ... your getting Dupuytren’s .. im young and pretty much said so what.

Well I have it in both of my hands. My left hand was so closed up I couldn’t use it anymore. I searched and searched for a good dr for over a year.

I just had the needle thing done to my thumb and Xiaflex to my ring and middle finger on my left hand.

Yep. It hurt. Yep it really hurt. It’s day two. I’d do it again in a second. After 20 yrs I actually can wash my hair and not just knuckle it. My fingers feel broke. I guess they should. Haven’t been open in years. The doctor was great. He also seemed peeved I and not he met the great hand surgeon of all time. James Hunter. Ha.

My hand really hurts today. I want to get my rt hand done tomorrow.

To see my left hand open. Made me cry.

Hi I would suggest you consider using a night splint. I was given advice after NA that I could stretch the hand for short periods up to 14 days after the procedure and after that not to force the fingers to straighten them as it may be detrimental and may retrigger the disease.

Yes, they made me a night splint and said to where it at nights for 3 months. I didn’t know of the stretching fingers back. Still swollen and bruised up good. I’m iceing it every other hr. My follow up with the Dr is Nov 8th. Thanks for the recommendations.

My Dr did say ... not to pick up anything heavier then a glass of water for 7 days.

I really like my doctor. It’s well worth searching for the one.

I just got a notice that my insurance is being cancelled. They are changing or doing away of the BCBS platinum plan. It’s good to Jan 1st but enrollment starts November 1st. I got all the codes used to make sure I’m covered for my right hand. Really pushing to have the rt hand done before Jan. It’s not a cheap procedure. Check and double check your insurance plan.

You might try within the first 14 days whilst watching TV. Sit on a soft lounge place your hand under your but palm facing down and gently apply pressure on the hand for a few minutes and rest . This technique was given to me after NA by a German Hand Surgeon.
