Radiation to just the nodule or include the cord?

I am starting radiation for my second time tomorrow. We are focusing only on the nodule at the base of the thumb, but I have a cord that runs through the webbing between my pointer finger and the thumb. Is that typical or should the cord typically be included in the area that received the RT?

Welcome to the forum teeps

The hypothesis on how RT works for DD is such that it has no effect on cords. However the target area around the nodule should be extended as a margin to cover disease that is not seen or felt, but may be present.

Hello,

Best practice is to include cords within the radiation target area. See the link below from this site where there are example photos that show the marking for cords and the treated area includes the cords.

The best study to date on RT treatment for DD comes from Prof. Seegenschmiedt's 2012 paper on the Phase III clinical study for such treatment. Here is a quote from this paper that might be helpful:

"Local RT was applied depending on the individual grade and extent of DD. It was common policy in our clinic to treat the whole afflicted area of the palm including all palpable and visible nodes and cords with sufficient distal and proximal (1–2 cm and lateral margins (1 cm)"

It is interesting that your treatment area is so small. In my case most of the palm plus a portion of the affected fingers were treated. But I had several nodules as well as a cord, not just one nodule.

A downside for treating a larger area is the cancer risk, which could discourage a second treatment. I suppose you are avoiding that problem with the small treatment area.

Just finished day 3. Just 7 more to go and then time will tell if it is effective :)
