I'm 47 year old, male, with garrods pads and very recently Duputryens.

I have it in my little finger, right hand, PIP joint. I had Xiaflex treatment a year ago and it has recurred with visible cord down into the hand - its at 90 degrees. I have a few other very small nodules on other fingers but they have been there for years.

Has anyone had any experience of a second Xiaflex treatment on the PIP joint? I am very keen not to have surgery if I can possibly avoid. I'd be happy to do another Xiaflex or NA and Xiaflex first.

I would welcome any thoughts or suggestions. I am Australia based but will travel worldwide to find the best Doctor.

I had the same problem, i.e. fairly quick recurrence, at the same joint (right PIP of the little finger) after a Xiapex treatment. You need to be aware that this is the worst of all joints with respect to treatment and recurrence. However you treat it, contracture will likely return after a fairly short time. The only means that I know of to delay recurrence is night splinting as SB already indicated.

I believe there is no problem to have a second collagenase injection , except that this is a very difficult joint, also for collagenase. I had my recurrence treated by needle fasciotomy (NA) and it worked out fairly well. Since then I am wearing a night splint and have been able to keep my fenger straight so far (but the NA was only 7 month ago).

My Xiaflex procedure was performed 2 1/2 years ago by a plastic surgeon who specializes in hands. So far, so good. He also does NA and full surgeries and chooses which procedure will be most successful. Before I decided on having him do it, I checked and he had performed about 70 successful Xiaflex procedures. If my other hand starts contracting or it returns to my treated hand, I will have the same doc use Xiaflex on it.
