Nodule - What To Do?

Well I'm back....

I had nodule on my left pinky PIP joint two years ago which rapidly progressed to a contracture. I had NA done on the finger soon thereafter and while there is and always will be some contracture the finger has held up pretty well to this point.

Just today I noticed what feels to be a nodule in my right hand. It's located just below my middle finger. There's absolutely no signs of any contracture of the fingers or pitting in the hand.

Given that ----- should I wait this out for a little while to see if this progresses. I recall that nodules in the fingers are some of the worst areas for treatment and therefore I saw a doctor fairly early on after discovering the nodule on the joint.

If I did go see a Doctor who specializes in Dupuytren's at this point what's going to be the likely recommendation....?

Re: Nodule - What To Do?

I think a doctor will say wait and see for just a new nodule. It's probably good advice, even with your history, as it usually progresses slowly. If the nodule is extra sore, maybe troublesome, you could try a steroid shot, but the results of that are mixed or dubious. Why not investigate where and who would do RT, in case it suddenly progresss with more nodules and cords? If you don't like RT then it's true, NA on cords in the palm causing contracture is usually more successful than on the fingers. You could look at some alternatives, massage, using magnesium or vitamin e oil, etc, but none are a proven treatment. I would map my hand, take a photo, do the same after a week or month, research treatment options, RT or NA, make contact with the doctors or add them to a list or my phone, then try to carry on as normal.

Thanks very much for your response...

I'm pretty obssessed with health related issues so I'll certainly stay on top of this. I saw two different doctors initially before having my NA completed, however I'm not sure how either one of them feels about RT as I was not a candidate given the location of my contracture.

From what I've read, RT can still be performed with a slight contracture so when/if things progress I'll be on it quickly. Guess I'm still struggling with how effective RT is in delaying the disease. Is it now fairly "widely accepted" as a standard course of treatment by most Dr's or is there still considerable debate on it's effectiveness?

Also ---- I live in Florida so would be interested in any personal recommendations of Dr's in the southeast that forum users have had a good experience with in terms of RT.

Much appreciated....

Re: Nodule - What To Do?

There are an increasing number of hand surgeons who are aware of the literature and practice for RT and DD. However they are probably still the minority. Also the predominant literature is still that from Seegenschmidt, Betz, and others out of Germany, despite growing use in the US and other European countries. This is available as formal publications or books with references including a control trial. The rate of success in slowing or halting active progression remains above 80-85%. We list some in Florida with experience on our treatment pages, http://www.dupuytren-online.info/radiotherapy_clinics.html, also Dr Diaz hand surgeon in Jupiter comes up highly recommended for early consult (not for RT) http://www.dupuytren-online.info/NA_list_North_America.html. Given your caution I would think a consult with her might be in order, along with the aforementioned wait and see plan.
