Dupuytrens, both hands
I live in Arizona (the greater Phoenix area) and have contracture of my little fingers both hands, the tip of my finger nearly touches the base of the finger it is so bent, very pronounced "cord" is present and I am desperate. Looking for any direction on best way to proceed. Self employed, no health insurance, 50 years old. Thank You

firenzi06; Needle Aponeurotomy or NA should be your first measure. Most surgeons are going to recommend radical surgery which is expensive and can involve months of physiotherapy. NA is an a simple 20 minute procedure and you have full use of your hand again within 3 days. There are a number of doctors that do this procedure in the USA but I don't know what it costs.
A good option is to take a weeks holiday in Paris. Dr Badois in Paris is very good. I have been to him twice. Cost is 300 Euros ($US360). In my case Dr Badois did my right hand at the beginning of the week and my left and the end of the week so I always had one useable hand.
Google Dr Badois and Needle Aponeurotomy and you will get a lot of information.

Re: Dupuytrens, both hands
I would start with Dr. Paul Zidel, he was a presenter at the Miami International Dupuytren's Symposium and is a former colleague of Dr. Eaton. Dr. Zidel is now in Phoenix. He can give you advise on whether NA is feasible to correct those bent fingers.
