I had both, surgery and Xiaflex. In my case Xiaflex was by far not as painful as surgery, no comparison really. You still can have the manipulation. If you explain your pain to the doctor before the manipulation, he will surely numb your finger accordingly. that's standard practice. Not going to the manipulation after you already had the injection with all its unpleasant side effects would be a waste of money and pain.

You don't really want the cord to snap without a block. It will likely be very painful. My Xiaflex procedure went pretty smoothly, it was done by a plastic surgeon 5 years ago who specializes in hands and was quite painless once the injections were done. Injections hurt like hell but I had full use of my long deformed hand within a week of the cord snap. My middle finger is just beginning to curl again, that's the nature of this scourge. I will likely have Xiaflex again if it gets very bad unless some better procedure comes along.
