Platelet-Rich Plasma (PRP) DD & Ledderhose

I've searched as much as I can online for any connection to DD and prior PRP treatments. (very basic PRP- blood must first be drawn from a patient. The platelets are separated from other blood cells and their concentration is increased during a process called centrifugation. Then the increased concentration of platelets is combined with the remaining blood and then injected into the patient to "stimulate" the body's healing response.) It was first used for ortho injuries, now promoted for just about everything under the sun.

Both my husband and I have DD and LD. He has a severe ankle injury and his podiatrist is suggesting PRP which she described as "stimulating the body's immune response, causing inflammation, causing the body to put down extra collagen, healing the tendon tear."

This seems to me to be the *exact* opposite of what one wants w/regards to DD or LD. I haven't been able to find any research as to how far the PRP blood can travel in the body and how close it stays to the injection site.

I was in the Xiaflex trials in 2007 and had PRP (oral surgery) in 2010. Prior to these two events, I had DD in one finger, I now have it in all 10 digits--(I had NA w/Dr. Eaton in 2005, 2009, 2011 and Dr. Denkler, 2009, 2014, Dr. Pess-2013,--last two less than 50% improvement) and LD in one foot . I've always attributed the rather fast increase in my disease to Xiaflex although there doesn't seem to be any clinical evidence. Now I'm wondering if it had something to do with PRP.

I'm interested in ideas, research, anything that will help my husband make a good decision. I did see one very small study showing mesenchymal stem cells (MSC’s) prepared from adipose tissue (fat) vs. regular stem cells *might* be helpful for DD.

However, my main question is--will PRP for whatever other purpose-- in this case his ankle-- *stimulate* DD?

Hi Diane,

I agree, there is nothing in the literature on PRP affecting Dupuytren disease. So I am only speculating: PRP is produced from your own blood and when it is applied you have locally more platlets but not overall in your body. Therefore I, personanlly, would not think that PRP is affecting Dupuytren's, except possibly when it is applied to a nodule or cord directly.

Besides that, in Germany warnings were issued about PRP produced by docotors themselves and not under controlled conditions in a blood bank. These warnings were issued as a safety measure and are not based on evidence. I guess most doctors recommending it will produce PRP themselves making some extra money with it. Personally I owuld be reluctant with it in general.
