Xiaflex and resulting significan skin tear--questions

Hello. New to this forum. My husband (age 64) had the Xiaflex treatment last week--injection(s) on Tuesday, then the "manipulation" on Thursday--his "pinky" finger had contracted to his palm--yes, it should have been treated sooner, but it was his right hand and the timing was never quite right. The manipulation resulted in a "tear" approximately the size of a nickel--a large, gaping wound. Immediately following his appointment we were sent downstairs to the physical therapy/rehab center for a custom splint. Hubs was fitted with a splint and told to wear it 24/7 until we return for our first follow-up tomorrow (Tuesday, January 23)--of course, removing it to change the dressing as needed. My question/concern is that his doctor has not scheduled any follow-up visits--other than the physical therapists--until 4 weeks post-procedure. Does this sound normal? This wound is quite significant. We are both paranoid of infection! I am taking every precaution I know of to ensure that everything is as sterile as one can get it in a home environment--as of this writing, the wound shows no signs of infection. I think he needs to be seen by a doctor--not just a physical therapist, before 4 weeks. Also, the doctor did not use silver nitrate or any meds on the wound when he dressed it. The therapist had to re-dress the wound when splinting as it was already saturated with blood (this was 2-1/2 hours after the procedure). I have read on this forum of silver nitrate being used on the resulting open wounds. Hubs is having me place Neosporin on the dressing every other day--we are changing the dressing every day--we changed it twice a day for the first two days. The doctor came highly recommended, but I think he is more of a carpal tunnel guy than Duputryn's. We live in Houston, Texas. Any advice/feedback would be greatly appreciated!

When I had Xiaflex done on my hand they did the injection, then I had to wait a week to go back for the manipulation. I think a week was too long but it seems that each doctor has his own ideas on that. Fortunately I did not experience any skin tearing.

Last month I had surgery on the same hand (same fingers actually!) and went back to the surgeon after 10 days to have the stitches removed and the wound inspected. At that time the surgeon gave me some gauze tape things that were impregnated with Neosporin, or Vaseline, or something similar and I had to change the dressing daily using these things to keep the wound moist and prevent infection. After a week or so the wound had closed up enough that daily dressings were no longer needed and I am well on the way to a complete recovery.

In your husband's case I would be calling the doctor to get an appointment to have the wound looked at - 4 weeks with no follow-up doesn't sound safe! In the meantime I'd keep Neosporin on the wound, changing the dressing at least daily.

I'm not a doctor, but I do know that infections are no fun!

Yes I agree with Alan, although it’s common to have skin tears which are left open to heal, some wound care should be done, even it it’s merely a look by an experienced eye. In the UK that would be a nurse, but it might be different for you. I would not wait for an infection to set in and see your GP just to get follow up care advice (for the wound). Hopefully within a week to 10 days you’ll be past the period of concern.

My finger ripped open during an NA procedure. It wasn’t pretty. The dr stitched it up and that was that.
