Dupuytren & boutonierre on DIP joint?

Does anyone have a boutonierre deformity on DIP joint? I have Dupuytrens on PIP joint of right pinkie and also a boutonierre deformity on DIP joint of same finger. I had NA on the PIP joint, but the contracture came back before a year, I plan to have the NA re-done followed by radiation therapy..... and wander if anyone has had experience/good results with a boutonierre involvement.

Same story with me. Surgery on the PIP joint only succeeded in freezing it at 45 degrees, following which the DIP joint hyperextended so now my fingers is a 'Z'. I'd very much like to have this fixed oralleviated but won't do another conventional surgery. While having NA on a thumb, I asked about possible release of the pinkie and the response was "maybe."

Jimh......I'm happy to find a fellow "boutonierre sufferer", the Z shape of the finger describes it perfectly, the hyperextension is pulling up so much on the DIP joint that it hurts, do you have that problem?.

That is one of the most amazing things about this forum, you don't feel alone and you can share your problems with others who have the same problems and who really understand you.

Jimh....which Dr. told you "maybe" on releasing the boutonierre? I sent pictures of my hand to Dr. Denkler (who is the one who did my first NA on that PIP joint two years ago) and he also said he would try to release it... I'm planning to do that in April 2010 and to follow it immediately with radiation therapy (by Dr. Tripuraneni at Scripps in La Jolla).....in the hopes of being more successful than the first time around and hoping it will not return.

Re: Dupuytren & boutonierre on DIP joint?

It doesn't hurt but it's uncomfortable and awkward. I recently had NA by Dr. Marco Rizzo at Mayo, and he gave me a "maybe" on the bent pinkie. I think he said he couldn't tell in advance if the problem was scar tissue from surgery, or a recurrence of contracture, or some combination of factors. Unfortunately he's "out of network" for me, my insurance only pays half, making it expensive to take a chance on a treatment that might not work. Nevertheless I'm considering it.

Jimh......If you did treatment at Mayo clinic I'm assuming you live near East Coast.... Dr. Denkler in Lakspur near San Francisco is more aggressive, and takes on cases others turn away......would that be a possibility for you? Don't know what insurance he takes.

When I did NA with him in 2007 I paid cash, but Blue Cross PPO reimbursed me almost 100%. I plan to re-do my NA with him on "z" shaped pinkie finger, but I'm lucky I know have Medicare which I didn't have before and I believe they will cover it..... I'll check into this beginning of next year, since just started today radiation on my other hand.

Re: Dupuytren & boutonierre on DIP joint?

I live in Minneapolis and Dr. Rizzo is at Mayo in Rochester, MN. We may switch our insurance to a plan that would cover Mayo. I've heard lots of good reports on Dr. Denkler but travelling to CA is expensive. Dr. Rizzo seems committed to the procedure, is not timid in performing it, and I think he will only become more effective (I don't know if he'd want to say "agressive") over time. So I think I'll be going back to him in the future.

Yes, I really want to hear Dr. Denkler's opinion, thanks.
