Here is an excellent discussion of Dupuytren's treatment options
Several things of interest to me.
1. As Dr. Eaton points out, it was the power of the internet and message boards like this one that helped to bring about the spread of needle aponeurotomy to the USA. The same thing appears to be happening now with radiation therapy.
2. Radiation is mentioned in this AAOS article. Amazing isn't it. I am seeing medical information websites (i.e. Mayo Clinic) comment on RT as a possible treatment option for DD. But for the most part the hand surgeons many of us have visited are oblivious about RT, likely because it just isn't part of their training and education.
3. It is hard to read much into the limited information but it sounds like there may be some actual advantages for collagenase over NA in some situations.
Anway, I thought the article was interesting.

Thank you. It was an interesting discussion. The little that was mentioned about fasciectomies supported my experience. They certainly weren't prepared to "throw surgery under the bus".
When asked, "What do you think the role of open surgery is? Is it still the gold standard or the procedure of choice"?
"Dr. Meals: When open surgery is done properly and the patient sees it through with therapy, the likelihood for a functionally significant recurrence of the contracture is small. Unless the contracture has a real tendency to re-form, the chances are that the patient will not need any additional treatment in that area. Open surgery clearly has a role, even in previously untreated Dupuytren".
The discussion was good and clearly there needs to be more.
