Has anyone used topical pirfenidone for Dupuytren's
Hi, Has anyone used topical pirfenidone for Dupuytren's? There is an 8% pirfenidone gel for scar treatment marketed in Mexico by the CellPharma firm under the name KitosCell. I have asked the company directly about this issue but they answer me that there are only in vitro results and not in-vivo. My question is whether anyone on this forum has used this gel at their own risk and what the results have been. Regarding oral Pirfenidone, you know that there is a lot of information in PubMed, including patients treated for pulmonary fibrosis with oral pirfenidone whose Dupuytren disease was cured at the same time. The way that KitosCell works is described here (inhibiting TGF BEta1): https://kitoscell.com/en/producto/ I translate into Spanish since it is in Mexico where Kitoscell is sold, perhaps there are more potential users (i live in Spain) Thanks IDS for this Forum, it helps a lot!!

Re: Has anyone used topical pirfenidone for Dupuytren's
Haven't used it but the issue with topical cremes is that very, very little passes through the skin barrier. In vitro results therefore have not much valence. For Dupuytren's topical crenes are typically useless (DMSO might work for arthritis but that's something else).
Are you aware that we have a Spanish forum as well https://www.dupuytren-online.info/forum_es/ ?

Thank you Wolfgang. PLease see this https://pubmed.ncbi.nlm.nih.gov/25533576/ "Pirfenidone gel in patients with localized scleroderma: a phase II study": The 8% pirfenidone gel application was well tolerated, and no side effects were detected. Conclusions: This is the first study on the therapeutic use of pirfenidone gel in localized scleroderma. It acts on both the inflammatory and the fibrotic phases. Considering its effectiveness, good safety profile and the advantage of topical application, pirfenidone is a treatment option in this condition.
Scleroderma is not DD but also TGF Beta1 is involved and both diseases have many things in common. Surely someone has tried pirfenidone gel for DD. In this case it would be very interesting to know the results!
