NA is an outpatient procedure (cost around $1000) to relieve contractures that keep you hand from opening fully. Risk is very low and you have immediate use of your hand after the procedure which takes about an hour. Possible to fly into an area, get the procedure, and fly home the same day. Dr. Denkler in Larkspur (just north of the Golden Gate bridge at San Francisco is excellent and you could take in the sights.

XRT is something to do at the early stages to prevent further progression of the disease. At latter stages XRT is not likely to provide any benefit. XRT in the USA for me was $9500 for CAT scan and XRT for second set of sessions. First set of XR treatments was done at Essen Germany and the cost was 400 Euros (about $600 US at the time). So if you do not have insurance coverage going to Germany is the least expensive option I could find.

A delay of even 6 months can make a big difference in the effectiveness of the XRT so don't delay any more than is necessary. There are two hospitals in Canada providing the XRT but you will need to speak French or have an intermediary who does. Personally I would treat both hands even if only one is showing outward signs of the disease.
