Dr. Keith Denkler at the Meeting of American Association of Plastic Surgeons

Dr. Keith Denkler of California continues to be a leading medical advocate for NA. He recently presented a study to the 88th Annual Meeting of American Association of Plastic Surgeons. It has been falsely claimed that the later stages of Dups contracture are not good candidates for NA. Denkler focuses on stage IV, showing that NA is just as powerful as OS even in these cases. He uses the words "subcutaneous fasciotomy" in place of Needle Aponeurotomy (NA).

I have met Keith, as I know a number of you have as well. He is a great guy and we are fortunate to have him fighting against the CHS's seemingly intransigent knee jerk reaction for OS every time, all the time. He is making progress with his fellow plastic surgeons as well. Have a look.

Thank you for the link to the abstract. Is the whole paper available anywhere?

When I had NA done by Dr. Denkler week before last, he was very excited about the opportunity to present his paper. He hopes to change the minds of his peers about NA. He submitted a paper last year to a different group and it was not accepted. I understood him to say that this group that accepted his paper is smaller but an elite plastic surgeon's group. I hope they are open to his attempts to educate them.

I'm educating my GP, my dermatologist, my dentist and any other person who may be in position of recommending treatment for Dupuytren's. I'm also educating friends and anybody else that I can about NA and other alternative therapies and that surgery is not the only option.

I, too, say "Thank you, Keith." for putting patients first and being an advocate for us.

Here is what Dr. Denkler said about the presentation he gave last week:

"The presentation was well received and some key docs were very impressed.
The tide is turning and that is good.
I am advocating NA as a first step, rather than surgery.
NA can be repeated and will also allow enzymatic treatment."

Dr. Keith is taking the right course by promoting NA as the "first step", rather than a replacement for Open Surgery. While Eaton told me that he thinks NA will prevent the need for surgery for >50% of new patients in their lifetime, this claim might be hard for long time surgeons to swallow. To simply say it's worth a try before going the usual OS route is a softer approach.

And in fact, that really is the case. Because of higher rates of recurrence in aggressive disease NA may run out of gas, forcing the patient up the ladder of treatment to more invasive measures. But that's the case in most treatment modalities anyway.
