Ideas/Thoughts on a few things

Was diagnosed with DC 6 years ago and i have watched it slowly turn my LH to a 10% contracture. Then noticed a small lump on bottom of foot (ledderhose) also have peyronie's disease (it doesnt really affect me). i started RT had my 1st session 2 weeks ago and i return in 4 weeks for next session. Maybe its me but it seems to be getting aggressive.So here is the pretentious question. If money isnt a object who is the best in the business to resolve my hand? would you wait and see what results you got from the RT? i am a TYPE A and want to fixed now kind of guy. My Doc said lets go thru 2 treatements THEN wait 3 months and have xiaflex then 1 more round of RT. I want my hand back to normal it doesnt affect me VERY Much but want ot get ahead of it. I want to go see a specalist who ONLY deals in these 3 things. Right now i am seeing a doc who only does RT for it and before that i was at a sports medicine guy who only does injections. Is there a GURU who specializes exclusively in these 3? Thanks

It’s highly unlikely you will find a radiologist who also does surgery. You might find a surgeon who does NA and/or Xiaflex as well as surgery and is sympathetic to RT. Your treatment plan seems a bit skewed, normally people have RT before a contracture, or have NA or Xiaflex to straighten a contracture and then have RT. There is no best overall of everything. ProfsS in Germany led the way with RT, the French doctors with NA, and surgery had been the gold standard for years. But with patient groups, forums, conferences etc the overall approach is changing and improving. If you read our website and forum you will see we advocate wait and see, RT early stages or prior to contracture, minimal intervention via NA or Xiaflex and surgery for severe contracture. There are other emerging choices which have less of an evidence base but anecdotally at least have some encouraging reports.

Unfortunately I have mostly bad news for you:

1. Dupuytren's is a genetic disease and so far there is no cure for it. There are various treatments that can improve the symptoms but none of them can heal this disease permanently.

2. There are no "Dupuytren specialists" that are offering all treatments. Some do collagenase injection, others do radiotherapy etc..

The good news is that this website is offering a lot of valuable information on Dupuytren and Ledderhose disease. You might start with reading https://www.dupuytren-online.info/dupuyt...ontracture.html and then continue with the related pages. And, of course, you have this forum for asking questions. Familiarize yourself with the details, getting informed is the best way to deal with this disease.

To add to the preceding replies, it may be worth trying to prevent your slight contracture from increasing, or perhaps reduce it slightly, by wearing a splint glove at night. Has been working for me for three years now, although I did also get RT on that finger, after NA and when it had already started to bend again.
