Pain management during manipulation following Xiaflex injection

I have significant contractor in both hands and have chosen to go with the Xiaflex treatment. I just finished with my second injection and underwent a manipulation two days ago. Because of the confluence and number of my cords involved, there is significant breakage that must occur.

I have to say that when my hand was extended, and the cords began to break, the pain was excruciating even though the physician had used a wrist block. I had to stop after three manipulations because of the pain (we were successful in obtaining significant extension).

Has anyone had experience with successful pain management during manipulation other or in addition to using a wrist block? I want to proceed with another manipulation, but I want to minimize the pain as much as possible so I’ll continue with the treatment.

With NA there was considerable pain with the numbing injections (maybe 5 or 6). However, the "manipulation" to cut the cords and obtain release caused zero pain or discomfort - literally I did not feel a thing. And the entire process was done in one shot - maybe 30-40 minutes from stem to stern.
