flojo: I think you're right on. I thought I knew my hands, but the nodules and tightening keeps happening. I think I have an aggressive DD, but believe my hand would be very contracted if I had not had RT. Fortunately (I think), mine does not affect the finger joints so far except from tightening in the palm. Weird! It is convenient for me to see Dr. Denkler week after next, so I plan a consultation with him. It will be interesting to hear what he has to say. It was 2 years ago that I had NA with him then RT 5 months later.
Hi Flora, I'll be very interested to hear how your appointment with Dr. Denkler goes.
Even though everyone is different and ours is such a weird disease...I would like to know what he thinks of the recurrence rate of Xiaflex (or maybe its too soon to know since Xiaflex is so new).....versus the recurrence rate of NA.
In my case, I've had NA twice on same finger and both times it came back after six months....don't know what to do next. I was also thinking of driving up to see him....

That's my lesson I have learnt, because Dupuytren affects NOT ALWAYS the whole hand !
Radiation is ONLY EFFECTIVE while acting against fibroblasts; if fibroblasts are not present or not active, radiation therapy can be a waste ! It's better to detect nodules & cords, then observe if they are really active by strict comparison of photographs or own palpation and TREAT ONLY if ACTIVITY of the disease is really proven !
Often times the THUMB will never have any functional problem even if Dupuytren is there ... so why treat, if there are no obvious symptoms or no disabling functional deficit ?
From my discussion with Prof. Seegenschmiedt, who treated me before for my LEDDERHOSE on both feet and now recently for ONE HAND, prophylactic radiation for "dormant disease" is not useful. That's why my other hand has not been irradiated parallel with the other hand ! However, a sufficient "safety margin" around active disease" is very important for radiation.
My Conclusions:
1. Keep REGULARLY track on your disease by palpation (e.g. every 3 months) 2. Perform FUNCTION TESTS like spreading your fingers or thumb, test grip etc. 3. Document carefully all procedures on your hands using a sketch or photocopy 4. Ask your physician, orthopedist, hand surgeon or radiotherapy doctor to give you copies of their procedures which they have done to complete your "history" ...

Radiation therapy - only in experienced centers ?!

I have rapidly growing DD in both hands. I have not had steroid injections or surgery yet. Am trying to avoid both. I am seeing Dr Delaney at MGH in Boston next week for possible RT-first visit.
Questions: how do I get insurance to cover RT if it can be done on me? .....

Who is Doctor Delaney ??
Has he or she experience in palpating DD and what RT protocol are they planning to use? Consider to go to an experienced center and doctor with a couple of years of clinical experience - that's important for the first round !!
Costs can be tremendous ! Compare it with costs abroad (example Germany) and you might want to take a week of vacation .... even with less costs !

My cost in San Francisco was over $8000 for the second round of XRT and I also had to pay a $1400 CAT scan fee. The cost for the first round of treatments in Essen, Germany was 400 Euros. You could have a very nice vacation in Germany depending upon your co-pay or deductible and be dollars ahead.
I had Dr. Denkler perform a NA procedure on my hands 2 months before visiting Dr. Seegenschmiedt in Essen and in spite of Dr. Denkler's experience with Dupuytren's he had not understood how far the disease had progressed in my second and non-dominant hand. That was were Dr. Seegenschmiedt's decades of experience was invaluable in preparing for adequate XRT of the affected areas of both hands.
But it is important to get the XRT as soon as possible wherever that is going to be. My own treatment was delayed by nearly a year as my GP, Dr. Dawn Motyka of Santa Cruz, failed to diagnose it properly and even tried injecting a saline solution as a treatment.
XRT may halt progression but it will not reverse it.

I live in Maine and Boston and Dr Delaney are the closest RT center listed on this website. I have not met or talked to Dr Delaney yet. His medical assistant had little info besides; she said he RTs " a couple of DDs per month", she did not know his protocol but said they use a electron machine.
Has anybody out there used Dr Delaney for RT for DD or LD? I quess I'll ask all these questions when I see him next week.
Thanks to all who responded for your support. Nobody in Maine seems to understand the crippling effects of DD, I see my hands just getting destroyed before my eyes.

I have visited Prof. Seegenschmiedt who practices in HAMBURG and I was fascinated by his diligence in examining both my hands and feet !
I got 2 series of RT on a linear accelerator within 3 months 5 sessions of 3 Gy each totalling 30Gy. My whole payment was 940 Euros !! And I got the best knowledgable doctor whom I could ask all my open questions ...
I would really recommend everybody on this forum to seek professional advice of an experienced doctor when considering RADIOTHERAPY !
VACATION in GERMANY especially in HAMBURG is great and even better to meet the "PROF" and alltogether its much cheaper than in the USA .
