Your experience with NA...

Hi there - So I have been in touch with Dr. Gary Pess in NJ and he believes I am a candidate for NA. I am so glad as the 2 doctors I saw locally haven't heard of NA and recommended surgery. I received the initial paperwork I need to send to the office and there was info about the local anesthesia and it tells me I will feel the needle and the "sawing" of the cord. I am not a wimp at all and have a high pain tolerance - but this makes me feel squeamish! For any of you who have done NA - how difficult is it - how about when the finger is pulled to break the cord? Has anyone had NA done with Dr. Pess? Thanks!

Hi Jackie. I do not have a very high tolerance for pain and don't like syringes or being operated on, and I had no problem at all and simply felt relieved and pleasantly surprised by how easy it was in my case. So chances are you will be fine and based on my experience you should feel just slight discomfort but no real pain, even after the anesthesia wears off. My only recommendation would be to really follow the instructions not to put strain on that finger for a couple of weeks, which is easy to forget when things seem normal but the healing must still complete under the skin. Did he mention collaganase injections (Xiaflex)? My hand surgeon, who is skilled in surgery, NA and collaganase inj, recommended Xiaflex in my particular situation. I am extremely pleased with the results. Mine was done three months ago.

I had NA done by Dr Denkler in Larkspur California first in March of 2007 followed by XRT, and then has a partial NA done with Xiaflex in July of 2013 followed by a more complete NA with additional Xiaflex injections in November of 2013.

For the NA in 2007 Dr. Denkler injected lidocaine as a local anesthetic and once this was done I had no feeling for the remainder of the procedure. In 2013 Dr. Denkler injected my hand at the wrist with licodaine and this immediately numbed my entire hand in less than 10 seconds. He then proceeded to do multiple Xiaflex injections.

The day after the Xiaflex was injected (which was done mid-morning) I returned in the afternoon to Dr. Denkler's office and he again injected my hand at the wrist to numb it completely. He then proceeded to break free the contractures and also did NA at the same time using a special bent needle to pull the skin away from the diseased tissue below.

His approach was so unique that a representative from Xiaflex was at his office to take pictures of the procedure. Dr. Denkler as able to do the combined procedure in less than an hour and the combination of the two techniques was far superior to either alone. There was no pain with his approach to numbing the entire hand with a single injection at the wrist. Dr. Denkler is a certified hand surgeon and so more aware than others as to the efficacy of this approach in reducing patient pain and making it easier for him to what is needed in terms of manipulation and use of the needle during the NA procedure.

As a squeamish person myself, I had similar concerns about NA. I had read on this forum that the pain from the lidocaine injections is similar to a bee sting that lasts for a few seconds and it was spot on. No big deal, and after that there was no pain whatsoever. As far as feeling the sawing goes, there was one point where there was a painless sensation where I was aware that the needle was rubbing across the cord fibers, but in a strange way I enjoyed the sensation, realizing that the cord I had learned to hate was being torn asunder.

All-in-all the experience was easier than a trip to the dentist for a filling.
