Trouble after NA followed by Xiaflex

Hi for the first time everyone. I had NA followed by an injection of xiaflex 5 weeks ago and my hand is still swollen
And I still can not make a fist. Fingers come down half way. Have been icing and doing Physio. DR out of town for 2 weeks
The deep pain and throbbing relentless. Any thoughts. I starting to think injection went into tendon or ligament

Hello and welcome. Sorry to hear about your problems and pain. 5 weeks sounds like a long time. Can you share more information about your condition and the procedures performed? Why was Xiaflex injected in addition to the needle procedure? Was your finger or were your fingers (which? the same?) pulled straight a) after the NA, b) after the injection? How much time between the NA and the injection?

Hello. I was to believe the surgeon was just going to inject the xiaflex but when he froze my hand and started digging
Away with a needle I notice the xiaflex was still on the counter. He straightens my fingers and I heard popping noises
Then he said I will put injection in to clean up anything left. He wrapped me up and I went home. I returned a week later
And he froze me up again and did something not sure what and re wrapped me and sent me home. He said he would call and set up Physio. Which he didn't. I had to myself. He said call me in 5 weeks if I have any questions. The ring finger and middle finger and baby finger were affected and quite contracted. My Physio lady said things are not normal but that's all ahe has said. I have deep pain and throbbing and burning 24/7 and I feels like the this hand will never close again. It just stops half way to fist and that's it.

I am just a fellow patient trying to educate myself, having been through one successful NA/PNF procedure and two rounds of apparently also successful radiation therapy. Based on what you have written, also about what the physio told you, I do not have the impression that you are in good hands with that doctor. Although he seems to have straightened your fingers successfully at first with the NA, it sounds to me like he has been experimenting on you and not taken care about you the way I would expect from my doctor. Hopefully others will share their views here, especially those who have been through Xiaflex injections, but if it was me I'd go see someone else quickly.

I would think you would do either NA or Xiaflex, not both! Another thing that I don't think people discuss much here is that Xiaflex results in an immune response. It seems likely that each time you have the treatment the immune response would increase, perhaps to the point that an allergic reaction would occur. If NA worked I would stop there. Sounds like not an experienced doctor.

Thanks for ur thoughts. Very concerned. Hand is nothing but a pain producing appendage right now. I'm sure this is not right or no one would want this procedure.

I have had Xiaflex injections twice and the first time the doctor had me wait a week before doing the manipulation and my hands became so swollen that the skin cracked open. The second time the manipulation was done 14 hours after the Xiaflex injections and the swelling was much less. The manipulation on day two I believe enabled the hand to remove the waste products from the Xiaflex more effectively and this reduced the swelling.

In both cases I used cold packs to chill my hand to minimize swelling but it was not enough.

Doing NA and then Xiaflex is wrong. NA can be used where Xiaflex fails and so it should be done at the same time the hand is being manipulated the day after the Xiaflex injections. The day after the injections the doctor can see where there is still impingement and then use NA and additional manipulation to break these other areas free.

With Xiaflex and NA in combination you will get better results and it only takes one day for the Xiaflex followed by manipulation and NA the second day. Least expensive and most effectiver treatment process.

I can only close my hand half way. Major Physio and I work in it hours every day. Will not close to grip
Doc says I need to work on it harder nothing he can do. May not come back.
I had no idea this was a possibility. I'm am still hoping this nightmare will resolve.
No one would do it if this was the way it went.

Really sorry to hear this is still not improving. As I wrote 2 1/2 weeks ago, if it was me, I would see a different doctor as soon as possible as this is not normal and the one you have been seeing seems neither competent nor trustworthy and is unlikely to help you in an unbiased way. Hope you can find someone competent in your area fast.
