How about we establish a database here of all of us that have had NA in the US and include the name of our health insurance providers and whether it was covered. That way, those of us who might be having problems getting the procedure covered won't have to start from scratch providing info. If we can find someone with the same ins. coverage who has been successful in getting coverage, it should make it much easier.

I suggest that you request coverage for the procedure now, before you have it done, even if you are certain they will turn it down. Otherwise, when and if you make an appeal, they can turn it down simply for the reason that you did not seek prior approval.
