PIP joint pinkie contracture questions-seeking advice/ informaion

I have had NA done on this hand three times, every 4-5 years with excellent results (thank you DR Charles Eaton). My latest contracture in my pinkie is more in the PIP area. More so than in the past. I have contacted a hand surgeon in South Dakota, where I reside, who does do some NA procedures ( but does not specialize in this, does more open surgery and the injections). The Dr is advising me to have open surgery TO REMOVE THE CORD, rather than NA. Dr is indicating that the NA will not be as effective in the PIP joint for several reasons. I am seeking information from others who have had NA to resolve contractures in the pinkie PIP area, their experience, satisfaction with the NA, and if any damage concerns (nerves, etc) were voiced about doing NA higher in the fingers than in the palm area. Thank you so much for your information. Oh, many years ago I monitored a Dup Forum and a Wolfgang ? Provided a lot of technical and sound information to inquiries. Is he still active? GHoek

Dr. Eaton ...2009 N.A. left pinkie 95 degree PIP contacture....

No pain at all Steroid injection into nodules by Dr. Eaton ..then the NA

My finger is still near perfect ...have worn a splint at night for all these years...

Also of note ... I have rather small hands and fingers....

This I believe answers your questions .... If I were you ... seek out the best NA doctor you can find ...

Dr. Denkler performed NA on my 95 degree PIP pinkie contracture in 2006. Followed up with 2-3 months pf PT, wearing a splint day & night app. 30 days, then at night for app. 3-4 years. Very successful. Not perfect but excellent overall and still holding up.

You are correct, NA is not as effective in the PIP joint as it is in MCP joint. That said--thoroughly research surgery for DD and the consquences and side effects, including acceleration of the disease-- a side effect of all interventions--and if you still think surgery is the best option for you, get the best surgeon you can find with lots and lots of experience.

I've had NA so many times, I've lost actual count-- --- 2005, 2009, 2011, 2013, 2014, 2015-- in the beginning it was just one finger, then in 2007 I had Xiaflex and now it's all fingers-- and thumb web space, both hands. In the beginning with Dr. Eaton I was able to go every 2 years but now it's every year although I'm pretty much maintaining by wearing night splints every night--I alternate and only wear my splint on one hand at a time.

If you choose NA--it's worth a trip to Dr. Denkler. I did have nerve damage with another physician-- PM for details.
