Hi,

I think you misread my post - I think his insurance company WANTED preauthorization but because there was non - refused the procedure.

Please correct me if I am wrong.

If you don't get pre-authorization than they can deny coverage based just on that, and ignore all of the issues. I suggest that you apply for pre-authorization then fight it, if denied.

Some insurance pays much higher amounts for out of plan surgery that is not in your home state.

I have no idea of the logic, but mine pays more for out of plan in Conn. than in NY City.

California HMO-Pacificare 2

Sorry all, I neglected to mention that my NA with Dr. Eaton was in May 2004. My pre-authorization request was denied in March, BEFORE NA, and then this appeal was denied in July.
